Sleep Problems

WHEN SLEEP FEELS IMPOSSIBLE AND NOTHING HAS FIXED IT, YOUR BRAIN MAY NEED A DIFFERENT KIND OF HELP.

Poor sleep isn't just exhausting. It affects your mood, memory, focus, relationships, and physical health in ways that compound over time. If you've tried sleep hygiene, medication, or therapy and still aren't sleeping well, the issue may be rooted in dysregulated brain activity that those approaches can't fully reach.

At BrainCare Performance Center Carlsbad, we focus on repetitive transcranial magnetic stimulation (rTMS) because it’s a non-invasive, drug-free treatment that works directly on the brain networks involved in sleep regulation. Many patients report improved sleep quality and duration as one of the first changes they notice during treatment.

Sleep Problems Q & A

Why does sleep feel like such a hard problem to solve?

Because sleep isn't just a habit, it's a brain state. Your ability to fall asleep, stay asleep, and reach restorative deep sleep depends on the coordinated activity of multiple brain networks. When those networks are dysregulated—often due to stress, trauma, depression, anxiety, or neurological factors—no amount of bedtime routine can fully compensate.

That's where brain-based treatment comes in.

What kinds of sleep problems do you treat?

We see patients dealing with a wide range of sleep disruptions, including:

  • Insomnia (difficulty falling or staying asleep)
  • Non-restorative sleep (waking up exhausted despite hours in bed)
  • Hypersomnia (sleeping too much, especially linked to depression)
  • Sleep disruption related to depression, anxiety, or PTSD
  • Sleep problems following a traumatic brain injury (TBI)
  • Sleep issues in autism or other neurodevelopmental conditions

Sleep problems rarely exist in isolation. Your clinician will evaluate both your sleep patterns and any co-occurring conditions to create a plan that addresses the full picture.

What are common symptoms of sleep problems?

  • Difficulty falling asleep, waking frequently, or waking too early
  • Sleeping too many hours but still feeling unrested
  • Daytime fatigue, brain fog, or difficulty concentrating
  • Mood changes—irritability, low motivation, emotional reactivity
  • Increased anxiety or depressive symptoms
  • Physical effects: headaches, weakened immune response, weight changes

How does rTMS help with sleep?

rTMS uses precisely targeted magnetic pulses to stimulate areas of the brain involved in sleep regulation and mood. This helps retrain dysregulated circuits, reducing hyperarousal, supporting natural sleep architecture, and strengthening the brain's ability to shift into and maintain restful sleep states.

Importantly, rTMS can improve sleep directly—not just as a side effect of treating depression or anxiety. Many patients report better sleep quality and duration within the first two weeks of treatment, often before other improvements become apparent.

How is sleep treatment approached at BrainCare Carlsbad?

Treatment depends on what's driving your sleep problems. For most patients, rTMS is the primary recommendation.

rTMS (Repetitive Transcranial Magnetic Stimulation)

rTMS uses gentle magnetic pulses to stimulate brain regions associated with sleep, mood, and emotional regulation.

  • Non-invasive and drug-free
  • Well-tolerated with minimal side effects
  • FDA-approved for Depression (a leading cause of sleep disruption)
  • Often one of the first benefits patients notice is improved sleep

Investigational TMS (MeRT)

For patients whose sleep problems are linked to a more complex neurological picture—such as TBI, autism, or other conditions—we may recommend MeRT® (Magnetic e-Resonance Therapy). MeRT combines TMS with qEEG brain mapping to create a highly individualized treatment protocol based on your unique brainwave patterns.

The right path depends on your underlying condition, your goals, and what your qEEG/EKG results show. Your clinician will walk you through both options.

What happens during care?

First appointment: testing
We begin with a quantitative electroencephalogram (qEEG) to map your brainwaves, along with an EKG to compare heart–brain timing. This baseline is completed for all patients, whether pursuing rTMS or MeRT.

Second appointment: results & plan
You'll review your qEEG/EKG findings with your clinician and discuss the best treatment path. If you move forward with rTMS, sessions run five days a week for two weeks (your first treatment block).

Ongoing care: two-week blocks
At the end of each block, we repeat a brief qEEG/EKG and review your progress. If symptoms and brainwave patterns are improving, we continue in two-week intervals until you and your clinician are satisfied with your results.

What does an rTMS session feel like?

You sit comfortably in a chair while the coil rests lightly against your scalp. Most people feel a gentle tapping sensation and hear clicking sounds during the pulses. There's no anesthesia, no downtime, and no restrictions; you can drive yourself home and return to your normal day.

Will insurance cover sleep treatment?

Insurance coverage for rTMS depends on the underlying diagnosis. When rTMS is treating depression (a common driver of sleep problems), it is often covered by insurance when clinical criteria are met. Coverage for other diagnoses varies. Our team will verify your benefits and explain any out-of-pocket costs before you begin. We also accept CareCredit for flexible financing.
